Correction to the 2018 Newly Effective Preauthorization Procedure Codes List

On 7/30/2018, Blue Cross and Blue Shield of Texas (BCBSTX) corrected the effective date for procedure code 31295 (nasal/sinus endoscopy, surgical) to no longer require preauthorization as of 2/23/2018. It previously indicated 2/23/2019. A corrected 2018 Newly Effective Preauthorization Procedure Codes PDF Document list has been posted to the BCBSTX provider website under Preauthorizations/Notifications/Referral Requirements.

This list applies to fully insured members for the plans listed below:

Blue Advantage HMO^SM and Blue Advantage Plus^SM HMO
Blue Choice PPO^SM
Blue Essentials^SM and Blue Essentials Access^SM
Blue Premier^SM and Blue Premier Access^SM
